Objetivo:Revisar a literatura a respeito do papel do cirurgião-dentista frente aos casos de abuso sexual infantil, abordando sua importância na identificação e no encaminhamento adequado das vítimas, além de discutir a necessidade de conscientização eimplementação de políticas públicasque abordem o tema.Revisão deliteratura:O ambiente odontológico é um ambiente propício para a identificação de sinais de abusosexual infantil, uma vez que o profissional tem contato direto com a saúde bucal e facial das crianças. Desempenha um papel fundamental ao observar e reportar esses sinais, contribuindo para a identificação precoce e o encaminhamento adequado das vítimas.Discussão:A literatura préviasugere a necessidade de fornecersubsídios teóricos, aprofundados no tema,para contribuir na ampliação do conhecimento nessa área. Sendo importantea conscientização e capacitação do cirurgião-dentista nesse contexto, bem como a criação de políticas públicas que visam umaabordagem multidisciplinarquecontribuaparaa conscientização, prevenção e a promoção de ações voltadas à proteção das crianças e ao combate do abuso sexual infantil.Conclusão:O cirurgião-dentista desempenha um papel fundamental na identificação e no encaminhamento adequado das vítimas de abuso sexual infantil. É essencial que esses profissionais sejam devidamente capacitados e conscientizados sobre a importância desse tema, além de estabelecerem uma rede de colaboração com outros profissionais da saúde e serviços especializados.


Aim:To review the literature regarding the role of the dental surgeon in cases of child sexual abuse, addressing its importance in identifying and properly referring victims, in addition to discussing the need for awareness and implementation of public policies that address the issue. Literature review:The dental environment is a favorable environment for identifying signs of child sexual abuse, since the professional has direct contact with the oral and facial health of children. It plays a key role in observing and reporting these signs, contributing to the early identification and proper referral of victims. Discussion:Previous literature suggests the need to provide theoretical subsidies, in depth on the subject, to contribute to the expansion of knowledge in this area. It is important to raise awareness and training of dentists in this context, as well as the creation of public policies aimed at a multidisciplinary approach that contributes to awareness, prevention and promotion of actions aimed at protecting children and combating child sexual abuse. Conclusion:The dental surgeon plays a key role in identifying and properly referring victims of child sexual abuse. It is essential that these professionals are properly trained and aware of the importance of this topic, in addition to establishing a collaboration network with other health professionals and specialized services.

ABSTRACT

Abstract Objective: To describe the challenges pediatric dentists face while caring for their patients during the pandemic. Material and Methods: A descriptive qualitative study was conducted with purposefully sampled pediatric dentists. Data were collected through in-depth, semi-structured interviews until the content of the collected data reached theoretical saturation. Data were transcribed verbatim, coded, and analyzed using content analyses. Results: Seven participants (four females and three males) between 29 and 50 years participated in the study. Three themes emerged from the analyses: Anxiety and fear; PPE (Personal Protective Equipment) and its impact on care delivery; and 3) Behavior management. Conclusion: Dental care delivery was challenging for pediatric dentists. They experienced high anxiety levels and modified their services according to the recommended guidelines while making accommodations to lessen patients' COVID-19-related anxiety. The additional mandated PPE use affected the communication between the dentists and their patients, affecting their dentist-patient bonding.

El siguiente texto reconoce la importancia de nuestra especialidad, y plantea la preocupación por el riesgo de infantilizar nuestro ejercicio al trabajar con niños: somos una profesión científica que debe ejercerse basada en altos estándares de profesionalismo, evitando actuar como niños sólo porque son nuestro sujeto material de estudio. Esto, sin olvidar que debemos adaptarnos al proceso de crecimiento y desarrollo bio-psico-social de nuestros pacientes


O seguinte texto reconhece a importância da nossa especialidade, e levanta a preocupação com o risco de infantilizar nosso exercício ao trabalhar com crianças: somos uma profissão científica que deve ser exercida baseada em altos padrões de profissionalismo, evitando agir como crianças apenas porque são nosso sujeito material de estudo. Isso, sem esquecer que devemos nos adaptar ao processo de crescimento e desenvolvimento bio-psico-social de nossos pacientes.


The following text recognizes the importance of our specialty, and raises concern about the risk of infantilizing our practice when working with children: we are a scientific profession that must be practiced based on high standards of professionalism, avoiding acting like children just because they are our material subject study. This, without forgetting that we must adapt to the process of growth and bio-psycho-social development of our patients.

ABSTRACT

Objetivo: A escala Children's Experiences of Dental Anxiety Measure (CEDAM) foi originalmente desenvolvida em Inglês para avaliar importantes aspectos da ansiedade odontológica em crianças. Os objetivos do estudo foram traduzir e realizar a adaptação cultural da CEDAM para o Português Brasileiro. Material e Métodos: A CEDAM consiste de 14 itens, medidos por escala Likert de 3 pontos, que indica a intensidade da ansiedade odontológica. O questionário foi traduzido para o Português Brasileiro, retraduzido para o Inglês, revisado por um Comitê de Especialistas e pré-testado em 10 escolares de oito a doze anos. Resultados: O Comitê Revisor de Especialistas comparou as versões original, traduzida (T1, T2) e retraduzida (BT1, BT2) e recomendou algumas mudanças a fim de obter uma boa compreensão dos itens. No pré-teste, somente a questão 8 não foi compreendida por uma criança, isto é, a versão traduzida foi bem compreendida por mais de 85% dos participantes. Conclusão: A versão brasileira da CEDAM foi culturalmente adaptada para a população avaliada de crianças.(AU)


Objective: The Children's Experiences of Dental Anxiety Measure (CEDAM) was originally developed in English to assess important aspects of dental anxiety for children. The aims of the study were to translate and perform the cultural adaptation of the CEDAM to Brazilian Portuguese. Material and Methods: The CEDAM consists of 14 items, measured by a Likert scale of 3 points, that indicates the intensity of dental anxiety. The questionnaire was translated to Brazilian Portuguese, back-translated to English, reviewed by an Expert Committee and pretested in 10 eight- to twelve-year-old schoolchildren. Results: The Expert Committee Review compared the original, translated (T1, T2) and back-translated (BT1, BT2) versions and recommended some changes in order to achieve good understanding of the items. In the pretest, only question 8 was misunderstood by one child, i.e., the translated version was well-understood by more than 85% of the participants. Conclusion: The Brazilian CEDAM was culturally adapted for the evaluated population of children(AU)

Abstract Aim: Polymorphisms in the COMT gene can alter enzymatic functions, raising levels of endogenous catecholamines, which stimulates beta-adrenergic receptors related to pain. This study aimed to evaluate whether a polymorphism in the COMT gene (rs4818) is associated with dental pain in children. Methodology: A cross-sectional study was conducted with a representative sample of 731 pairs of children and parents randomly selected from a population-based sample of eight-year-old children. Reports of dental pain was evaluated using a question directed at the parents and self-reported pain using the Faces Pain Scale - Revised. Dental caries experience was determined using the Decayed, Missing, and Filled Teeth (DMFT) index. For genetic analysis, DNA was obtained from oral mucosa epithelial cells of 352 children randomly selected from the initial sample. Results: Children with the CC genotype had higher odds of reporting moderate to intense pain than those with the GG genotype (OR=3.60; 95% CI=0.80-16.20; p=0.03). These same children had greater odds of parental reports of pain (OR=1.93; 95% CI=0.91-4.08; p=0.02). Moreover, lower schooling of parents/guardians and caries experience in the primary dentition were significantly associated with greater odds of a parental report of dental pain (OR=2.06; 95% CI=1.47-2.91; p<0.001; OR=6.26; 95% CI=4.46-8.78; p<0.001). Conclusions: The rs4818 polymorphism of the COMT gene is associated with dental pain. Children with the C allele are more likely to report higher levels of pain. Clinical Relevance: Even though the experience of pain is subjective and multifactorial, this study raises the hypothesis that there is a genetic predisposition to dental pain that should be considered in clinical practice.

ABSTRACT

Objetivo: Avaliar a eficácia de um dispositivo de estimulação termomecânica (Buzzy®) em relação à dor, medo e ansiedade durante anestesia local em crianças. Materiais e métodos: Estudo realizado no período de maio de 2018 a julho de 2019, com crianças de 7 a 11 anos, sem experiência prévia envolvendo anestesia nos últimos 2 anos e que necessitassem de tratamento odontológico (extração, restauração ou endodontia) sob anestesia local em molares decíduos. A amostra foi randomizada em grupo controle, que recebeu anestesia convencional, e grupo intervenção, que recebeu anestesia com Buzzy®. Os níveis de ansiedade, medo e percepção de dor de ambos os grupos foram verificados por meio de: Venham Modified Picture Test (VPTM); frequência cardíaca; Escala Comportamental Venham; Faces Pain Scale ­ Revised (FPS-R) e Face, Legs, Activity, Cry, Consolability (FLACC). Resultados: A maioria das crianças (55%) apresentou baixa ansiedade antes e depois do tratamento (P<0,05). A aceitabilidade das crianças ao Buzzy® foi de 100% e a maioria (90%) gostaria de usar novamente. Discussão: O aparelho testado é uma ferramenta interessante para complementar as técnicas de manejo durante as consultas, tendo em vista a excelente aceitabilidade e interesse por parte dos pacientes e familiares. Conclusão: Este estudo demonstrou que o uso da estimulação termomecânica é viável na clínica odontológica, devido ao seu fácil uso e boa aceitabilidade no meio clínico, além de não apresentar riscos em seu uso.


Aim: To evaluate the effectiveness of a thermo mechanical stimulation device (Buzzy®) in relation to pain, fear and anxiety during local anesthesia in children. Materials and methods: Study carried out from May 2018 to July 2019, with children aged 7 to 11 years, without previous experience involving anesthesia in the last 2 years and who needed dental treatment (extraction, restoration or endodontic) under local anesthesia in deciduous molars. The sample was randomized into a control group, which received conventional anesthesia, and an intervention group, which received anesthesia with Buzzy®. The levels of anxiety, fear and pain perception of both groups were verified using: Come Modified Picture Test (VPTM); heart rate; Behavioral Scale Come; Faces Pain Scale ­ Revised (FPS-R) and Face, Legs, Activity, Cry, Consolability (FLACC). Results: Most children (55%) had low anxiety before and after treatment (P<0.05). The acceptability of the children to Buzzy® was 100% and the majority (90%) would like to use it again. Discussion: The tested device is an interesting tool to complement management techniques during consultations, in view of the excellent acceptability and interest on the part of patients and family members. Conclusion: This study demonstrated that the use of thermo mechanical stimulation is feasible in the dental clinic, due to its easy use and good acceptability in the clinical environment, in addition to not presenting risks in its use.

ABSTRACT

Técnicas de manejo comportamental são debatidas com frequência no âmbito odontológico. Dentre elas, a presença/ausência dos responsáveis legais na sala durante o atendimento do paciente é, ao mesmo tempo, simples e desafiadora de ser aplicada. O aumento do desejo dos pais em permanecer no consultório durante o atendimento traz consigo a importância de discutir os conflitos éticos e legais do uso dessa técnica. A revisão de literatura proposta teve como base os descritores "Pediatric Dentistry", "Child Behavior" e "Parents" e seus sinônimos, nas bases de dados Cochrane Library, LILACS, PubMed/MEDLINE, SciELO e Scopus, nos últimos 5 anos. Os resultados apontam divergências no estabelecimento de uma regra a ser seguida. Alguns trabalhos relatam que a presença de acompanhantes durante consultas odontológicas representa um importante apoio emocional para pacientes, resultando em melhor manejo de comportamento. Em contrapartida, a técnica de separação pode apresentar benefícios por consolidar a relação profissional-paciente. Contrapondo vantagens e desvantagens observadas na permanência e na separação dos acompanhantes em seus aspectos éticos (Código de Ética Odontológico) e legais (Código Civil, Estatuto da Criança e do Adolescente e Código de Proteção e Defesa do Consumidor), a literatura científica assinala que a maioria dos acompanhantes e crianças expressa o desejo de não serem separados durante o atendimento. Pacientes considerados incapazes não têm como exercer autonomia nas decisões relacionadas à assistência odontológica. Para os adolescentes, a conquista da autonomia ocorre conforme a maturidade do indivíduo. A incorporação dos responsáveis legais na tomada de decisão divide responsabilidades e traz legitimidade às condutas terapêuticas


Behavioral management techniques are frequently debated in the dental field. Among them, the presence/absence of legal guardians in the room during patient care is, at the same time, simple and challenging to apply. The increased desire of parents to remain in the office during care brings with it the importance of discussing the ethical and legal conflicts of using this technique. The proposed literature review was based on the descriptors "Pediatric Dentistry", "Child Behavior" and "Parents" and their synonyms, in the Cochrane Library, LILACS, PubMed/MEDLINE, SciELO and Scopus databases, over the last 5 years. The results point to divergences in establishing a rule to be followed. Some studies report that the presence of companions during dental appointments represents important emotional support for patients, resulting in better behavioral management. On the other hand, the separation technique can present benefits by consolidating the professional-patient relationship. Contrasting advantages and disadvantages observed in the permanence and separation of companions in their ethical (Dental Code of Ethics) and legal aspects (Civil Code, Child and Adolescent Statute and Consumer Protection and Defense Code), the scientific literature points out that most companions and children express the desire not to be separated during care. Patients considered incapable cannot exercise autonomy in decisions related to dental care. For adolescents, achieving autonomy occurs according to the individual's maturity. The incorporation of legal guardians in decision-making divides responsibilities and brings legitimacy to therapeutic behaviors

ABSTRACT

Aquellos casos en que se excede la fórmula dentaria normal primaria de 20 dientes o definitiva de 32 dientes se conocen como dientes supernumerarios o hiperodoncia. Los supernumerarios se pueden presentar de manera única o múltiple y de forma heteromórfica o eumórfica. El supernumerario más prevalente es el mesiodens, se da más en hombres, en dentición permanente y en el maxilar. Su etiología no está confirmada, pero puede deberse a una hiperactividad de la lámina dental. El tratamiento varía de acuerdo a cada paciente según si está afectando a la dentición normal o no y el desarrollo que tengan las estructuras dentarias. El siguiente reporte de caso habla de un paciente de 6 años, no sindrómico, en donde se presentó con un mesiodens erupcionado y se encontraron más supernumerarios gracias a la radiografía complementaria.


Those cases in which the primary normal dental formula of 20 teeth or the final one of 32 teeth is exceeded are known as supernumerary teeth or hyperdontia. The supernumeraries can be presented in a single or multiple way and in a heteromorphic or eumorphic way. The most prevalent supernumerary is the mesiodens, it occurs more in men, in the permanent dentition and in the maxilla. Its etiology is not confirmed, but it may be due to hyperactivity of the dental lamina. The treatment varies according to each patient depending on whether it is affecting the normal dentition or not and the development of the dental structures. The following case report talks about a 6-year-old, non- syndromic patient, who presented with an erupted mesiodens and more supernumeraries were found thanks to the complementary radiography.

ABSTRACT

Entre as perdas precoces de um elemento dental ocorre em grande frequência a do primeiro molar permanente, que normalmente tem o seu irrompimento por volta dos 6 anos de idade, e são propensos a serem acometidos por lesões cariosas que se não cuidadas precocemente pode-se levar a perda do elemento dental. Há também outros agentes que ocasionam a perda do elemento como o trauma de baixa intensidade, tais como, os cálculos dentários, os hábitos bucais deletérios e principalmente a falta de motivação de higiene oral. Diante disto, este trabalho visa a apresentação de um caso clínico de um paciente de 9 anos de idade, sem alterações sistêmicas, com grande destruição coronária no dente 26. Os exames de imagens foram colaboradores para diagnóstico e definição da extração. Foi realizada a exodontia em campo aberto. O paciente foi instruído quanto à higiene bucal e futuramente encaminhado para um tratamento ortodôntico. Conclui-se que o Cirurgião Dentista tem papel fundamental no diagnóstico e tratamento dessas alterações dentais, enfatizando a importância do tratamento restaurador de lesões cariosas precocemente, remoção de qualquer agente irritante de baixa intensidade e na motivação da higiene bucal(AU)


Among the early loss of a dental element, the loss of the first permanent molar occurs very frequently, which usually has its eruption around the age of 6 years, and they are prone to be affected by carious lesions that, if not treated early, can if it leads to loss of the dental element. There are also other agents that cause the loss of the element, such as low-intensity trauma, such as dental calculi, deleterious oral habits and especially the lack of motivation for oral hygiene. Therefore, this work aims to present a clinical case of a 9-year-old patient, without systemic alterations, with great coronary destruction in tooth 26. The imaging exams collaborated for the diagnosis and definition of the extraction. The extraction was performed in an open field. The patient was instructed about oral hygiene and later referred for orthodontic treatment. It is concluded that the Dental Surgeon has a fundamental role in the diagnosis and treatment of these dental alterations, emphasizing the importance of an early restorative treatment of carious lesions, removal of any low-intensity irritant and in the motivation of oral hygiene(AU)

Objetivo: Avaliar se a hospitalização na gestação pode influenciar na condição bucal do filho no terceiro ano de vida. Métodos: Estudo longitudinal com bebês de gestantes internadas e acompanhadas no setor da Obstetrícia de um Hospital Escola em Pelotas, RS, Brasil. Os dados referentes a hospitalização e ao parto foram coletados do prontuário hospitalar e no terceiro de vida do filho (a) de um questionário aplicado a mãe e do exame bucal da criança. Cada agravo bucal foi avaliado com critérios específicos, por uma examinadora calibrada e analisado no programa IBM SPSS Statistics com 5% de nível de significância. Resultados: Participaram 20 díades mãe-filho (a). Alterações da oclusão acometeram 95% das crianças, sendo a mordida aberta anterior (MAA) a principal. Ainda, 25% das crianças apresentaram opacidades demarcas e/ou hipoplasia do esmalte, sendo significativamente maior em filhos de mães mais jovens e 20% tinham cárie da primeira infância (CPI), estando relacionada à ausência de creme dental fluoretado e à qualidade da higiene bucal. Conclusão: O reflexo mais evidente da hospitalização na gestação na saúde bucal no terceiro ano de vida do filho (a) foi a oclusão alterada, especialmente a MAA.(AU)


Objective: To assess whether hospitalization during pregnancy can influence the child's oral condition in the third year of life. Methods: Longitudinal study with babies of pregnant women hospitalized and followed up in the Obstetrics sector of a Teaching Hospital in Pelotas, RS, Brazil. Data referring to hospitalization and childbirth were collected from the hospital records and in the child's third of life through a questionnaire applied to the mother and the child's oral examination. Each oral condition was evaluated with specific criteria, by a calibrated examiner and analyzed in the IBM SPSS Statistics program with a 5% minimum significance level. Results: 20 mother-child participated. Occlusion alterations affected 95% of the children, with anterior open bite (AOB) being the main. Still, 25% of the children had opacities and/or enamel hypoplasia, which was significantly higher in children of younger mothers, and 20% had early childhood caries, which is related to the absence of fluoride toothpaste and the quality of oral hygiene. Conclusion: The clearest reflection of hospitalization during pregnancy on oral health in the third year of the child's life was altered occlusion, especially the AOB.(AU)

Objetivo: Realizar una revisión bibliográfica de la literatura sobre las buenas prácticas en salud bucal, durante los primeros 1000 días de vida, el cual abarcó el periodo de gestación y los dos primeros años del niño. Materiales y métodos: Se realizó una revisión de la literatura científica desde febrero a marzo de 2022, para lo cual se utilizó las bases de datos Pubmed y Scopus. Los criterios de inclusión abarcaron artículos originales y revisiones sistemáticas (con o sin metanálisis) publicados entre 2016 y 2022. Se emplearon estrategias de búsqueda usando palabras clave, en inglés, derivadas del MeSH. Además, se revisó la literatura gris para definir algunos conceptos clave. Resultados: Se seleccionaron 72 artículos científicos y cuatro referencias de la literatura gris (una guía de práctica clínica y tres reportes). Los temas identificados se categorizaron en: buenas prácticas de higiene bucal, buenas prácticas de alimentación de la gestante y el infante, hábitos parafuncionales, y visitas al odontólogo. De acuerdo a los resultados, las recomendaciones clave para promover buenas prácticas de salud bucal durante este periodo crucial incluyen el uso de pasta dentífrica con flúor para la higiene dental una vez que erupcionan los dientes, evitar el consumo de azúcar libre, fomentar la lactancia materna en vez del uso del biberón y las visitas al odontólogo desde antes del nacimiento. Conclusión: Las buenas prácticas en salud bucal durante los primeros 1000 días de vida influyen significativamente en la salud oral y general del niño, por lo que es necesario adoptar medidas preventivas desde la gestación, enfocadas en la higiene oral y nutrición. La colaboración de un equipo de salud multidisciplinario, durante el embarazo y los dos primeros años de vida, es vital para mejorar la salud bucodental y general de la población.


Objective: To conduct a bibliographic literature review on current good practices in oral health during the first 1000 days of life, encompassing the gestation period and the initial two years of the child. Materials and methods:A scientific literature review was conducted in February and March 2022, utilizing the Pubmed and Scopus databases. The inclusion criteria encompassed original articles and systematic reviews (with or without meta-analysis) published between 2016 and 2022. Search strategies incorporating English keywords derived from MeSH were employed. Additionally, the grey literature was reviewed to define some key concepts. Results:72 scientific articles, and four references from the grey literature (one clinical practice guide and three reports) were selected. The identified themes were categorized as follows: good oral hygiene practices, good feeding practices for pregnant women and infants, parafunctional habits, and dental visits. Based on the findings, the key recommendations for promoting good oral health practices during this crucial period include the use of fluoride toothpaste for oral hygiene once teeth erupt, avoidance of free sugar consumption, encouragement of breastfeeding over bottle-feeding, and early dental visits before birth. Conclusion:Good oral health practices during the first 1000 days of life significantly impact the child's oral and general health, necessitating preventive measures starting from gestation, with a focus on oral hygiene and nutrition. The collaborative efforts of a multidisciplinary healthcare team during pregnancy and the initial two years of life are vital for enhancing both oral and general health outcomes for the population.

Objetivo: Comparar las terapias pulpares de dientes temporales en pacientes según la evidencia en la literatura. Metodología: Revisión bibliográfica en la literatura científica, en las siguientes bases de datos: PUBMED, MEDLINE, BVS, BBO Odontología, IBECS y Web of Science. Los descriptores seleccionados fueron: pulpotomía y pulpectomia, combinados en la siguiente estrategia de búsqueda: (pulpotomía) AND (pulpectomía). Resultados: Se encontraron 65 artículos, seleccionados por título y resumen. Después de una lectura minuciosa, solamente 23 artículos cumplieron con el criterio de inclusión que fuera un ensayo clínico, estudio longitudinal (cuasiexperimental o cohorte), no se excluyeron artículos duplicados y 13 que no tenían relación directa con el tema. La muestra final totalizó 7 artículos para la revisión integradora. La evidencia es limitada por el número de estudios que cumplieron con los criterios. Conclusiones: La contribución de este estudio sobre el tema es aumentar el conocimiento con respecto a las terapias pulpares; contribución que requiere ser mejorada o detallada en estudios posteriores; por tal motivo no se puede hacer una conclusión definitiva respecto a cuál de los tratamientos pulpares es mejor, pero nos indica una tendencia para desde el punto de vista clínico poder considerar los tratamientos pulpares como una alternativa adecuada para mantener el diente temporal en la cavidad bucal con evidencia científica.


Objective: To compare pulp therapy of primary teeth in patients according to the evidence in the literature. Methodology: Literature review in the scientific literature, in the following databases: PUBMED, MEDLINE, BVS, BBO Dentistry, IBECS and Web of Science. The descriptors selected were: pulpotomy and pulpectomy, combined in the following search strategy: (pulpotomy) AND (pulpectomy). Results: 65 articles were found by selecting by title and abstract. After a thorough reading only 23 articles met the inclusion criteria of being a clinical trial, longitudinal study (quasi-experimental or cohort), duplicate articles and 13 that were not directly related to the topic were not excluded. The final sample totaled 7 articles for the integrative review. The evidence is limited by the number of studies that met the criteria. Conclusions: The contribution made on the subject is to increase knowledge regarding pulp therapies, which needs to be improved or detailed in further studies, for that reason we cannot make a definitive conclusion on which of both pulp treatments is better, but it gives us a tendency and from a clinical point view to be able to consider pulp treatments as an adequate alternative to maintain the primary tooth in the oral cavity with scientific evidence.

El schwanoma es un tumor neural que se origina en las células de Schwann presentes en la vaina nerviosa cuya localización más frecuente a nivel oral es el dorso de lengua, siendo mucho más frecuente en adultos. En población pediátrica la presentación es muy rara siendo lengua su ubicación más frecuente, mientras que a nivel palatino solo se conocen 11 reportes en menores de 16 años. El objetivo de este reporte es presentar un caso clínico inusual de schwanoma palatino en un niño chileno de 12 años de edad, quien fue derivado con diagnóstico clínico presuntivo de carcinoma mucoepidermoide. El caso se complementó con radiografía panorámica, Conebeam, y luego biopsia incisional. Finalmente, se diagnosticó schwanoma palatino variedad sólida. Es importante considerar al schwanoma como diagnóstico diferencial en casos de tumores palatinos, en especial en casos pediátricos ya que se pueden presentar similares características clínicas a tumores de glándula salival.


The Schwannoma or neurilemmoma tumor originates from Schwann cells present in nerve sheaths. At oral level, its most frequent location is at the back of the tongue, being much more frequent in adults. In the pediatric population, this occurrence is rare and most frequently found in the tongue. At palatal level, only 11 reports are known in children under 16 years of age. The aim of this report is to present an unusual clinical case of palatal schwannoma in a 12-year-old Chilean boy, referred with a clinical diagnosis of mucoepidermoid carcinoma. The case was complemented with panoramic radiography and Cone beam, for its subsequent incisional biopsy. Finally, a solid palatal schwannoma was diagnosed. It is important to consider schwannoma as a differential diagnosis in cases of palatal tumors, especially in pediatric cases, since they can present similar clinical characteristics to salivary gland tumors.

Resumen Objetivo: El cementoblastoma es una neoplasia odontogénica benigna de origen mesenquimático relativamente poco frecuente que se asocia a las raíces de las piezas dentarias. Se considera que es la única neoplasia verdadera de origen cemental. El objetivo del presente trabajo es describir el caso clínico de un cementoblastoma de grandes dimensiones y realizar una revisión bibliográfica a fin de establecer pautas de diagnóstico y tratamiento, señalando sus diferenciales con patologías de similares características como la displasia cemento ósea y el fibroma cemento osificante. Caso clínico: Se describe el caso de un paciente pediátrico que presenta un aumento de volumen de considerables dimensiones asociado al primer molar permanente mandibular. Teniendo en cuenta las características clínicas y radiográficas, y con un diagnóstico presuntivo de cementoblastoma, se realizó la extirpación quirúrgica de la lesión junto con la pieza dentaria asociada, confirmándose el diagnóstico inicial por medio del estudio anatomopatológico.


Abstract Aim: Cementoblastoma is a relatively infrequent benign odontogenic neoplasia of mesenchymal origin that is associated with the roots of teeth. It is considered to be the only true cemental originated neoplasm. The objective of this article is to describe the clinical case of a large cementoblastoma and to carry out a bibliographical review to establish diagnostic and treatment guidelines, pointing out its differences with pathologies of similar characteristics, such as the cemento-osseous dysplasia and the cemento-ossifying fibroma. Clinical case: The case of a pediatric patient who presents a considerable increase in the volume associated with the mandibular first permanent molar is described. Considering the clinical and radiographic characteristics, and with a presumptive diagnosis of cementoblastoma, a surgical extraction of the lesion along with the associated tooth was performed, confirming the initial diagnosis by an anatomopathological study.

Os traumatismos dentários representam um problema de saúde pública devido à alta prevalência e impacto psicossocial, sendo muito frequentes em crianças podendo resultar em necrose pulpar e culminar na rizogênese incompleta de dentes permanentes. Nestes casos de traumatismos em dentes permanentes com rizogênese incompleta e necrose pulpar, o tratamento mais indicado pela literatura é a apicificação, sendo um tratamento complexo e longo. Considerando a importância deste tema, o objetivo deste trabalho é relatar o caso de um paciente com traumatismo dental e necrose pulpar, que acarretou a interrupção do desenvolvimento completo e adequado do ápice dentário, sendo necessária a intervenção por meio da técnica de apicificação e o acompanhamento a longo prazo. O caso trata-se de uma menina em que um traumatismo dentário acarretou fratura de um incisivo central superior com formação radicular incompleta, comprometendo tanto a estética quanto a vitalidade do dente. Foi realizado o tratamento endodôntico com apicificação e posteriormente ao plug apical de MTA. O dente foi restaurado definitivamente com resina composta e realizado acompanhamento do paciente. Nas consultas de acompanhamento houve ausência de sintomatologia dolorosa e satisfação da paciente com a aparência atual. Sendo assim, a apicificação apresenta-se como uma ótima alternativa para dentes com rizogênese incompleta e necrose pulpar, embora longo houve uma melhora estética e satisfação do paciente.


Dental trauma represents a public health problem due to its high prevalence and psychosocial impact, being very frequent in children and can result in pulp necrosis and culminate in incomplete root formation of permanent teeth. In these cases of trauma to permanent teeth with incomplete root formation and pulp necrosis, the most indicated treatment in the literature is apexification, which is a complex and long treatment. Considering the importance of this topic, the objective of this work is to report the case of a patient with dental trauma and pulp necrosis, which caused the interruption of the complete and adequate development of the dental apex, requiring intervention through the apexification technique and follow-up. long-term. The case is about a girl in which a dental trauma resulted in a fracture of a maxillary central incisor with incomplete root formation, compromising both the esthetics and the vitality of the tooth. Endodontic treatment was performed with apexification and then apical MTA plug. The tooth was definitively restored with composite resin and the patient was followed up. In the follow- up consultations, there was no painful symptomatology and the patient was satisfied with her current appearance. Thus, the apexification presents itself as a great alternative for teeth with incomplete root formation and pulp necrosis, although in the long run there was an aesthetic improvement and patient satisfaction.


Los traumatismos dentales representan un problema de salud pública debido a su alta prevalencia e impacto psicosocial, siendo muy frecuentes en niños y pudiendo dar lugar a necrosis pulpar y culminar en la formación incompleta de la raíz de los dientes permanentes. En estos casos de traumatismos en dientes permanentes con formación radicular incompleta y necrosis pulpar, el tratamiento más indicado en la literatura es la apexificación, que es un tratamiento complejo y largo. Considerando la importancia de este tema, el objetivo de este trabajo es relatar el caso de una paciente con traumatismo dentario y necrosis pulpar, que causó la interrupción del desarrollo completo y adecuado del ápice dentario, requiriendo intervención a través de la técnica de apexificación y seguimiento. a largo plazo. Se trata de una niña en la que un traumatismo dental provocó la fractura de un incisivo central maxilar con formación radicular incompleta, comprometiendo tanto la estética como la vitalidad del diente. Se realizó tratamiento endodóntico con apexificación y posterior taponamiento apical con MTA. El diente fue restaurado definitivamente con resina compuesta y el paciente fue sometido a seguimiento. En las consultas de seguimiento, no había sintomatología dolorosa y la paciente estaba satisfecha con su aspecto actual. Así, la apexificación se presenta como una gran alternativa para dientes con formación radicular incompleta y necrosis pulpar, aunque a la larga se produjo una mejoría estética y satisfacción de la paciente.

Las pulpectomías en molares primarios están indica-das en casos de diagnóstico de pulpitis irreversible o necrosis y reabsorción radicular mínima o nula. Son tratamientos laboriosos y extensos, que sólo pueden ser llevados a cabo en pacientes colaboradores. En búsqueda de simplificar esta terapéutica y mejorar su eficacia, se propone la mecanización de la pre-paración de los conductos de molares primarios. Diversos autores aseguran que esta técnica opti-miza el tiempo clínico y mejora la calidad del trata-miento, obteniendo gran aceptación en la literatura actual. Pulpectomies in primary molars are indicated in cases of irreversible pulpitis or necrosis with mini-mal or no root resorption. They are laborious and ex-tensive treatments, which only can be carried out in cooperative patients. In order to simplify this therapy and improve its effectiveness, the mechanization of root canal preparation is proposed. Several authors assume that this technique optimizes preparation time and improves the quality of treatment, obtaining great acceptance in the current literature. We report the sequence of procedures, resolution, and controls of two pulpectomies with mechanized instrumenta-tion in primary molars (AU)

Abstract Dental Specialties Centers (CEOs) are clinics representing part of the secondary oral health care system of the Brazilian Unified Health System (SUS). Pediatric dentistry is not a requirement for service accreditation. However, the CEO of the Federal University of Rio Grande do Sul (CEO-UFRGS) has provided dental care for children aged 3-11 years since 2017. The utilization rates of health services are influenced by absenteeism. Therefore, evaluation of absences for dental appointments is of primary importance. This study aimed to evaluate referral characteristics, absenteeism, and resolubility of pediatric dentistry appointments at the CEO-UFRGS. This retrospective cross-sectional study was conducted at the Dental Teaching Hospital of the university and analyzed secondary data from referrals and medical records. From August 2017 to December 2019, 167 referrals and 96 medical records were reviewed, from which data of individual variables related to the referral process and treatment were collected. Data were collected by a single trained examiner and analyzed using SPSS software. The main reasons for referral to secondary care were dental caries and pulpal or periapical diseases associated with difficult-to-manage behavior. An absenteeism rate of 28.1% at the first pediatric dental visit and a resolution rate of 65.6% were found. Binary logistic regression analysis indicated that each day of waiting for specialized care resulted in a 0.3% of higher chance of absence for appointment. This also increased the chance by 0.7% in child who attended the first visit to complete treatment, suggesting that the waiting time for attendance was associated with absenteeism and resolubility. Public policies that expand providing child dental care in secondary care are recommended to improve access to and resolubility of services.

Abstract This study aimed to evaluate the influence of cleft width on dental arch symmetry of children with unilateral cleft lip and palate. Forty-one children were subjected to impression preoperatively (T1; mean age = 0.31 ± 0.07 years) and postoperatively (T2; mean age = 6 .73 ± 1.02 years). Eighty-two digitized dental casts were analyzed by stereophotogrammetry software. The cleft palate width was measured in the anterior (P-P'), middle (M-M'), and posterior (U-U') regions. Also, the following measurements were obtained: anterior intersegment (I-C') and intrasegment (I-C); total intersegment (I-T') and intrasegment (I-T); cleft-side (C'-T') and non-cleft-side (C-T) canine tuberosity. Paired t test and Pearson correlation coefficient were applied (α = 5%). Cleft width had the following means: 10.16 (± 3.46) mm for P-P', 12.45 (± 3.00) mm for M-M', and 12.57 (± 2.71) mm for U-U'. In the longitudinal analysis, I-C' had a significant reduction, while the other measurements significantly increased (p < 0.001). Asymmetry was verified in the following analyses at T1: I-C' vs. I-C and I-T' vs. I-T (p < 0.001); at T2, only in I-C' vs. I-C (p < 0.001). At T1, P-P' vs. I-C' (r = 0.722 and p < 0.001), P-P' vs. I-T' (r = 0.593 and p < 0.001), M-M' vs. I-C' (r = 0.620 and p < 0.001), and M-M' vs. I-T' (r = 0.327 and p < 0.05) showed a positive and significant correlation. At T2, there was a correlation between M-M' and I-C' (r = 0.377 and p < 0.05). In conclusion, the anterior and middle cleft widths influenced palatal asymmetry in the first months of life, while middle width influenced residual asymmetry.

Abstract Objective: To translate and perform the cross-cultural adaption of the CD:H scale for use in Paediatric Dentistry in Brazilian Portuguese language. Material and Methods: The translation and cross-cultural adaptation of the CD: H was carried out in four stages: 1) translation of the instrument; 2) reverse translation (back translation); 3) cross-cultural adaptation, and 4) face validation. Face validation consisted of the evaluation of 30 subjects from the target population. A pilot study was conducted with 15 children aged 5-10 years treated at a university dental clinic and their drawings were analysed by two dentists. Data were analysed using Stata 12.0. Results: In the face validation, most items were understood; however, some words were changed, and terms were included to identify the dental environment. Good reproducibility was obtained: inter-examiner reliability was 0.9647 and intra-examiner reliability was 0.9619 for examiner A and 0.8260 for examiner B. Conclusion: The Brazilian version of the CD:H scale is a useful tool for dentists, helping identify children's emotions and being enjoyable for them.

ABSTRACT Objective: To evaluate the role of music as a distraction technique to minimize anxiety during dental care in children aged 7 to 9 years when submitted to local anesthetic procedure. Method: This study was carried out in the Dentistry Module of the State University of Southwestern Bahia, located in the city of Jequié, Bahia, Brazil. It was performed through the application of the Facial Image Scale, measurement of heart rate and blood pressure with the aid of the digital sphygmomanometer at two different times during pediatric dentistry care with and without the influence of classical music. Data were tabulated in the Statistical Package for Social Sciences software, version 21.0, expressed as medians and interquartile range. For inferential statistics, the Shapiro-Wilk normality test was used and the analysis of the effect of music was calculated using the paired T-tests and Wilcoxon, considering a significance level of 95%. Results: The sample consisted of 7 children, mostly male, with a median of 8 years of age. Regarding the psychophysiological variables, no significant variations were observed compared to before and after in the groups with music and without music. Conclusion: In this study, no differences were found regarding the reduction of anxiety in the group in which music was used. Additional studies with a representative sample are needed.


RESUMO Objetivo: avaliar o papel da música como técnica de distração para minimizar a ansiedade durante o atendimento odontológico em crianças de 7 a 9 anos de idade quando submetidas a procedimento anestésico local. Métodos: Este estudo foi realizado no Módulo de Odontologia da Universidade Estadual do Sudoeste da Bahia, situado na cidade de Jequié - BA. Foi realizado a partir da aplicação da Facial Image Scale, mensuração da frequência cardíaca e pressão arterial com auxílio do esfigmomanômetro digital em dois momentos distintos ao longo do atendimento em odontopediatria com e sem a influência da música clássica. Os dados foram tabulados no software Statistical Package for Social Sciences, versão 21.0, expressos como medianas e intervalo interquartílico. Para a estatística inferencial, utilizou-se o teste de normalidade Shapiro-Wilk e a análise do efeito da música foi calculada utilizando os testes T-pareado e o Wilcoxon, considerando nível de significância de 95%. Resultados: A amostra foi constituída por 7 crianças, pertencendo sua maioria ao sexo masculino, com uma mediana de 8 anos de idade. Em relação às variáveis psicofisiológicas, não foram observadas variações significativas em comparação ao antes e depois nos grupos com música e sem música. Conclusões: Neste estudo não foram encontradas diferenças no que diz respeito à redução da ansiedade no grupo em que a música foi utilizada, havendo necessidade de estudos adicionais, com amostra representativa.
